Mental Health Coordinator - Darwin.

The Mental Health Coordinator:

  • Provide Carers in the Darwin region with access to flexible support options. This includes assistance in their caring role and support to improve health and well-being through social and economic participation in the community.
  • Provides peer support in both individual and group settings that includes the provision of high quality, accessible and appropriate information, access to respite and support and referral to a range of support services.
  • Coordinates the implementation of group activities for Carers including respite, carer support, education and training, community and capacity building, particularly with relevance to the NDIS.
  • Participates in regional networks to share and receive information in the sector, also advocating and reporting on systemic issues.
  • Maintains responsibility for the Carer Peer Mentoring Program.

Peer Support Worker - Darwin

The Peer Support Worker

  • Provides Carers in the Darwin region with access to flexible support options. This includes assistance in their caring role and support to improve health and well-being through social and economic participation in the community.
  • Engages directly with Carers over the telephone or face-to-face, assessing their eligibility for services and responding to their unique and diverse caring situations. Peer Support is provided to Carers in both individual and group settings.
  • Provides high quality, accessible and appropriate information, access to respite and support and referral to a range of support services.
  • Provides upskilling and supervision of carers who will provide peer support within a mentoring framework to other carers.
  • Facilitates and presents to groups in an adult learning setting.

Mental Health Peer Support Worker - Katherine

  • Provide Carers in the Katherine region with access to flexible support options. This includes assistance in their caring role and support to improve health and well-being through social and economic participation in the community.
  • Engages directly with Carers over the telephone or face-to-face, assessing their eligibility for services and responding to their unique and diverse caring situations. Peer Support is provided to Carers in both individual and group settings.
  • Peer Support includes the provision of high quality, accessible and appropriate information, access to respite and support and referral to a range of support services.

Support Worker - Darwin

The Support Worker:

  • Assists care recipients in Centre based, Flexible Respite and Cottage respite environments
  • Provides quality care that enhances and promotes the quality of life for the care recipient
  • Delivers services to the care recipient as per the care plan 
  • Ensures that all services are delivered within a consumer directed care framework 

Carer Counsellor - Darwin

Reference: DCC   Closing date: COB Friday 2nd February 2018

The Carer Counsellor will:

  • Focus on enhancing the quality of life for carers through the provision of short term counselling
  • Target the provision of counselling to the specific needs of carers. This includes developing other modes of counselling to support carers access to short term counselling
  • Ensure counselling is integrated and coordinated with other services and forms of support by facilitating access to these, as appropriate to individual needs and circumstances.
  • Design, implement and evaluate the counselling delivered.

Support Worker - Katherine (casual)

The Support Worker - 

  • assists care recipients in Centre based and Flexible Respite environments
  • provides quality care that enhances and promotes the quality of life for the care recipient
  • deliver services to the care recipient as per the care plan
  • plans and coordinates activities within day to day activities of Centre Based and Flexible respite programs

Under the guidance of the Team Leader / Coordinator - Aged Care Services the support worker will ensure that all services are delivered within a consumer directed care framework
